The postural shift is dramatic. As the uterus expands and the baby grows, a woman’s center of gravity shifts forward and downward. To compensate, many pregnant women develop an exaggerated lumbar lordosis (increased inward curve of the lower back) that places enormous stress on the lumbar vertebrae, sacroiliac joints, and supporting musculature.
Relaxin changes everything. Beginning in the first trimester, the body produces relaxin — a hormone that softens and loosens the ligaments of the pelvis to allow it to expand during delivery. This is essential for birth but has a cost: the same ligamentous laxity that prepares the pelvis for delivery also makes it more vulnerable to joint dysfunction, misalignment, and the associated pain syndromes.
The sacroiliac joints and sacrum are central. The sacrum, the triangular bone at the base of the spine, serves as the keystone of the pelvic structure and is the point at which the spine transmits its load to the lower extremities. The sacroiliac joints on either side connect the sacrum to the iliac bones of the pelvis. When these joints are misaligned or restricted, the entire pelvic bowl is affected — with consequences not only for the mother’s pain and comfort but potentially for the space available to the growing baby.
What Is the Webster Technique?

The Webster Technique is a specific chiropractic analysis and adjustment protocol developed by Dr. Larry Webster, founder of the International Chiropractic Pediatric Association (ICPA). It is designed to reduce sacral subluxation and sacroiliac joint dysfunction in pregnant women, with the goal of restoring normal pelvic balance and function.
The technique involves two components:
- Sacral Adjustment A specific, gentle adjustment to the sacrum is applied to correct misalignment and restore normal movement to the sacroiliac joints. This reduces tension in the pelvic ligaments and musculature, restoring a balanced pelvic environment.
- Round Ligament Release A gentle contact to the abdomen addresses tension in the round ligaments, the ligaments that support the uterus and connect it to the front of the pelvic wall. Round ligament tension can contribute to uterine constraint (uneven tension on the uterine walls), which may affect fetal positioning.
Together, these two components work to create a more balanced, spacious, and functional pelvic environment for both mother and baby.
The ICPA and the Evidence Base
The ICPA has been the primary organization supporting research into the Webster Technique and chiropractic care during pregnancy. Their research efforts have included practitioner surveys, clinical case series, and observational studies examining outcomes in pregnant women receiving Webster Technique care.
A frequently cited study published in the Journal of Manipulative and Physiological Therapeutics (Pistolese, 2002) examined 112 cases in which the Webster Technique was applied during the third trimester for breech presentation, finding that 82% of cases showed successful change to vertex (head-down) position. This figure has been widely referenced in the chiropractic community.
An important clarification: The Webster Technique is not classified as a “breech turning” technique, and chiropractors are not claiming to manually turn babies. The mechanism is indirect: by reducing pelvic imbalance and uterine constraint, the technique creates a more symmetrical intrauterine environment — and babies, given sufficient space and freedom of movement, naturally tend toward the optimal vertex position. The practitioner does not manipulate the baby.
The Webster Technique is appropriate throughout pregnancy, not only in the third trimester for positional concerns.
